Miliary tuberculosis (MTB) is a rare, severe form of tuberculosis where bacteria spread throughout the body, creating millet seed-like lesions. While TB typically affects the lungs, penile tuberculosis is extremely uncommon, especially within urogenital infections. A recent case involved an immunosuppressed transplant recipient diagnosed with MTB presenting as a penile lesion. After further examination the man’s respiratory tract tested positive for Mycobacterium, and penile tissue also tested positive. The patient underwent a 12-month antibiotic regimen. The penile lesion worsened initially, requiring surgical removal of dead tissue. While his infection cleared after ten months, the source of the infection remains unknown, raising the possibility of contamination from the transplanted kidney.
